In checking the box below I agree that Uptown Yoga, Inc. is in no way responsible for the safekeeping of my personal belongings while I attend class. I understand that classes at may be physically strenuous and I voluntarily participate in them with full knowledge that there is risk of personal injury, property loss or death. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against Uptown Yoga, Inc. or its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.

LIABILITY WAIVER:

I understand that Uptown Yoga, Inc. locations are public facilities in which contagious viruses may be present. I agree that I will not hold Uptown Yoga, Inc. liable for any illness which may be contracted by visiting their public facilities and being in proximity with others, and for any allergic reactions to airborne remnants of cleaning products within the studio.

I understand that both Uptown Yoga, Inc. and its affiliated instructors are not diagnosing, prescribing or treating me or any illness I may have. I agree to consult a doctor or medical professional about a state of pathology, dysfunction, or pain, and to advise my instructors about such concerns. I understand that prior to beginning any exercise routine, I should consult with a medical professional.

I, my heirs or legal representatives, forever release, waive, and discharge Uptown Yoga, its employees, owners, instructors, managers and agents from and against any and all claims I may have now or claims that may arise related in any way, directly or indirectly, to the use of the public facilities owned or managed by Uptown Yoga, including without limitation claims related to negligence, theft, or other acts, and I hereby agree to indemnify and hold harmless Uptown Yoga, its agents, owners, employees, managers and instructors for claims from third parties caused, directly or indirectly, by my use of the Uptown Yoga facilities. I have read this Liability Waiver and fully understand its contents. I voluntarily agree to the terms and conditions stated herein.

I agree and acknowledge that I am signing this Liability Waiver as a condition to and as consideration for my use of the Uptown Yoga facilities, and as such, Uptown Yoga, Inc. may rely on these statements.
